Neman Grodno is one of only a handful of Belarusian clubs to have never been relegated from the top division since the Belarusian Premier League was established in 1992. The club was founded in 1964 in Grodno, a city on the Neman River, from which it takes its name. Over the decades the team changed its name several times, playing as Khimik Grodno from 1973 and Neman-Belcard Grodno from 1999, before reverting to its historic name in 2002.

Neman's closest brush with the national title came in 2002, when the team finished the regular season in second place before losing a championship play-off to BATE Borisov. Earlier, in the first year of Belarusian independence, the club won the Belarusian Cup (1993), and the feat was repeated twice more in the following decades, including in the 2024–25 season, when Neman defended the trophy by defeating Torpedo-BelAZ Zhodino 3–0 in the final — the club's third Belarusian Cup title.

Those cup successes opened the door to regular appearances in European qualifying rounds. Particularly memorable was the 2023–24 season, when despite finishing ninth in the league, Neman advanced to the Conference League qualifiers after clubs above them were disqualified over match-fixing allegations — the club made the most of the opportunity, eliminating Liechtenstein's Vaduz and then Malta's Balzan to reach the second qualifying round. Neman's home matches are played at the Neman Stadium in Grodno, holding just under 8,500 spectators — one of the oldest and most recognisable football venues in western Belarus.
